PET SUPERMARKET SWEEPSTAKES

These are the Official Rules for the Spot® Pet Supermarket Sweepstakes ("Sweepstakes"). The Sweepstakes is sponsored by Spot Pet Insurance Services, LLC ("Spot"), 990 Biscayne Blvd., Miami, FL 33132.

No purchase is necessary to enter or win. Making a purchase will not increase the odds of winning.

1. Eligibility.

1.1. Open to Legal Residents of Certain States. The Promotion is open to legal residents in the following states where a Pet Supermarket is located: AL, CA, FL, GA, KY, NC, SC, TX. The residents must be: (a) 18 years of age or have reached the age of majority in their jurisdiction of residence at the time of entry, whichever is greater, and (b) legally able to work in the United States. All of the remaining forty-two (42) states and the District of Columbia are excluded from the promotion. No purchase is necessary.

1.2. Spot Excluded. Employees and agents of Spot, its affiliates, subsidiaries, advertising and promotion agencies, and any entity involved in the development, production, implementation, administration, or fulfillment of the Sweepstakes (collectively referred to as "Sweepstakes Entities") and their immediate family members and/or those living in the same household of such persons, whether related or not, are not eligible to enter the Sweepstakes.

1.3. No Other Sponsorship. The Promotion is not sponsored, endorsed, administered by, or associated with Pet Supermarket.

2. How to Enter.

2.1. Promotion Period.

  • Each drawing for a prize under the Sweepstakes will run weekly starting 6/26/2023 and ending 9/17/2023. The weekly Sweepstakes Periods are as follows:            

Week 1: 6/26/2023 -7/2/2023            

Week 2: 7/3/2023 - 7/9/2023

Week 3: 7/10/2023 - 7/16/2023

Week 4: 7/17/2023 - 7/23/2023

Week 5: 7/24/2023 - 7/30/2023

Week 6: 7/31/2023 - 8/6/2023

Week 7: 8/7/2023 - 8/13/2023

Week 8: 8/14/2023 - 8/20/2023

Week 9: 8/21/2023 - 8/27/2023

Week 10: 8/28/2023 - 9/3/2023

Week 11: 9/4/2023 - 9/10/2023

Week 12: 9/11/2023 - 9/17/2023

2.2. Entry.

  • Each of the twelve (12) weekly Sweepstakes begin from 6/27/2023 through 9/17/2023.

  • To enter, complete one of the below options during the Sweepstakes Period:

A. VISIT SPOT’S DISPLAYS OR CHECK OUT SPOT’S FLYER’S PROVIDED AT CHECK OUT AT ANY PARTICIPATING PET SUPERMARKET. During the Sweepstakes Period visit your local Pet Supermarket in an eligible state and scan the QR code on a store display or flyer provided at check out and provide Spot and complete a quote for pet insurance. As a part of the quote, you must provide your first name, last name, zip code, and email address in addition to your pet’s species, breed, age, and pet name, click “Select Coverage”, to complete the quote process and be eligible to enter. Limit one (1) quote per person during the Sweepstakes Period.

B. ALTERNATE METHOD OF ENTRY: You may also enter by sending an email to spotpsmsweepstakes@spotpetins.com. You must provide a valid email address, phone number, address, and state of residency. No purchase or quote necessary. Limit one (1) entry per person during the Sweepstakes Period.

  • Entrants are only eligible for one of the twelve (12) separate sweepstakes promotions and only for the corresponding weekly drawings after entry until the end of the Sweepstakes.

  • By confirming completing a quote or providing the information for an alternate entry, you are agreeing to these Official Rules. No purchase necessary. Limit one (1) entry per person/per quote.

2.3. Restrictions.

  • Limit one (1) entry during the Sweepstakes Period. Spot will disqualify anyone found to have submitted multiple entries. In addition, Spot will not consider ineligible, illegible, or incomplete submissions sent to Spot’s address.

2.4. Conditions.

  • Entrants agree to be bound by these Official Rules and the decisions of Spot, which are final and binding on all matters relating to the Sweepstakes. Spot is not responsible for any typographical or other errors in printing the offer, administration of the Sweepstakes or the announcement of the prizes, or for lost, late, misdirected, damaged, incomplete or illegal entries.

2.5. Participant's Consent.

Entry constitutes consent to participate in Sweepstakes and for Spot to obtain, use, and transfer the entrant's photo for Sweepstakes administration purposes. By participating in the Sweepstakes and/or accepting any prize, entrants grant Spot permission to use their name, likenesses, and image in connection with the Sweepstakes for purposes of advertising, publicity, and promotion purposes, without further compensation to the entrant, unless prohibited by law.

3. Drawing.

Each unique quote completion, or alternate entry during the Sweepstakes Period enters you for a chance to win. Spot will choose one (1) winner from all eligible weekly entrants through random drawings and notify the winners via email or phone within five business days. If a winner is not eligible or if Spot cannot contact the applicable entrant, such entrant’s prize will be forfeited, and an alternate winner may be selected. The odds of winning depend on the number of eligible entries received. Spot will choose a total of 12 winners throughout the Sweepstakes Period.

4. Prizes.

4.1 Prizes. Each winner will win a Pet Supermarket gift card valued at FOUR HUNDRED DOLLARS ($400.00). The terms and conditions of each gift card can be found at spotpet.com/psm-terms. Only one prize allowed per person.

4.2 Expenses and Taxes. Prize winners are responsible for any expenses incurred in connection with the prize. No substitution or transfer of prizes is permitted, except that Spot reserves the right to substitute a prize of equal or greater value if an offered prize is unavailable. All federal, state, and local taxes on prizes are the sole responsibility of the winner. Prize details not set forth herein are the sole discretion of Spot.

4.3 Legal Documents. If requested, winners must execute and return an affidavit of eligibility, liability release, and (where legal) publicity release within three days of notification. Failure to do so may result in disqualification or forfeiture of the prize and the prize awarded to an alternate entrant.

5. General Rules.

5.1. Applicable Laws. All federal, state, and local laws and regulations apply. The Sweepstakes is void where prohibited by law. The prize winner may receive an IRS 1099 form for the value of their prizes. Entrants agree to be bound by these Official Rules and the judges' decisions, which are final and binding on all matters relating to Sweepstakes.

5.2. No Responsibility. Spot, and each of its respective affiliates, subsidiaries, parent corporations, and their respective officers, directors, shareholders, employees, and agents, are not responsible for any incorrect or inaccurate entry information, theft, tampering, destruction, or unauthorized access to, or alteration of entries; printing or other errors; and any entries or mail which are late, lost, delayed, incomplete, misdirected, stolen, mutilated, illegible, postage-due, or any combination of the above.

5.3. Spot's Rights. Spot can void any entrant's entry who it believes has attempted to tamper with or impair Sweepstakes administration, security, fairness, or proper play. Spot reserves the right to modify, suspend or terminate the Sweepstakes if necessary due to circumstances beyond Spot's control. If the Sweepstakes is terminated early, the judges will select the winners from all eligible entries received at the time of termination.

6. Governing Law and Disputes.

6.1. Governing Law. This Promotion is governed by and subject to the laws of the United States and the state of Florida without giving effect to the conflict of laws rules thereof.

6.2. Disputes. Except where prohibited, the entrant agrees that any and all disputes that cannot be resolved between the parties, and all claims and causes of action arising out of or connected with the Sweepstakes or any prize awarded, or the determination of winner shall be resolved individually, without resort to any form of class action. Further, in any such dispute, under no circumstances will entrant be permitted to obtain awards for and hereby waives all rights to claim punitive, incidental, or consequential damages or any other damages, including attorneys' fees. Each entrant further waives all rights to have damages multiplied or increased.SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU.

6.3 Limitation of Liability: Entry into this Sweepstakes constitutes an agreement to release and hold harmless Spot and Pet Supermarket, and its subsidiaries, representatives, affiliates, partners, advertising and promotion agencies, successors, agents, assigns, directors, employees, and officers against and from any and all claims, liability, illness, injury, death, litigation, loss, or damages that may occur, directly or indirectly from participation in the Sweepstakes. Additionally, Spot is not liable for any 1) winner accepting, possessing, using, or misusing of any awarded prize or any portion thereof; 2) any type of technical failure in the Sweepstakes; 3) the unavailability or inaccessibility of any transmissions, phone, or internet service; 4) unauthorized intervention in any part of the entry process or the Sweepstakes; 5) electronic error or human error in the Sweepstakes administration or the processing of entries.

7. Privacy.

By entering this Sweepstakes, you also agree to Spot and Pet Supermarket’s use of your personal information as described in: (a)Spot’s Privacy Policy at spotpet.com/psm-terms; and (b) Pet Supermarket’s Privacy Policy at https://www.petsupermarket.com/privacy-policy.html. Your information will be shared for marketing purposes by Spot and Pet Supermarket.
