Sam's Club Gift Card Terms and Conditions

Limit one per customer.

Only available for Sam’s Club members who purchase a new Spot plan through the designated Sam’s Club member enrollment flow after the promotion start date.

Available to residents in AL, AR, AZ, CA, CO, CT, DC, DE, FL, GA, HI, IA, ID, IL, IN, KS, LA, MA, ME, MI, MN, MO, MS, MT, NC, NH, NJ, NM, NV, NY, OH, OK, OR, PA, RI, SC, SD, TN, UT, VT, WA, WI, WV, and WY based upon purchase of a policy and subject to the following condition:

The Sam’s Club gift card is eligible to be claimed thirty-two (32) days after the purchase of the applicable pet insurance policy, provided the policy is active and the policyholder’s account is in good standing.

To claim your gift card, you will receive an email with instructions from Spot Pet Insurance with a link to claim your Sam’s Club gift card. The ability to redeem the gift card and the link to claim your gift card will expire after one hundred and eighty (180) days from policy purchase.

While rewards are automatically issued upon meeting all eligibility requirements outlined in these Terms, if you do not receive an email from Spot with instructions to claim your Amazon gift card 32 days after your policy purchase date, you must contact SpotOps@spotpet.com within sixty (60) days of the date the reward was due. Failure to do so may result in forfeiture of the reward, at Spot’s sole discretion.

Pet insurance policy renewals are not eligible for the promotion.

Gift card is not valid or combinable with any other gift card promotion for policy purchase from Spot Pet Insurance Services, LLC.

Spot reserves the sole and exclusive right to withhold the reward in cases where fraudulent activity or abuse of the promotion is suspected, including but not limited to those that are submitted artificially or generated by the use of software.
